29 CFR 1910.134(c)(1): A written respiratory protection program that included the provisions in 29 CFR 1910.134(c)(1)(i) - (ix) with worksite specific procedures was not established and implemented for required respirator use: (Construction Reference 1926.103)    St. Anthony Hotel, Floors 3 and 4: On or about February 3, 2015 employees were required to wear 3M and MSA half-mask tight fitting respirators while performing demolition tasks where inhalation hazards from asbestos and silica were present, and the employer did not establish a written respiratory protection program or implement provisions of the program, such as but not limited to medical evaluations, fit tests, or selection and maintenance of respirators.

29 CFR 1926.1101(d)(3): The employer of employees who were exposed to asbestos hazards did not comply with applicable protective provisions to protect such employees:    St. Anthony Hotel, Floors 3 and 4: On or about January 7, 2015, employees were exposed to respiratory hazards from demolition and cleanup processes of debris with asbestos containing materials, such as damaged pipe insulation and discarded duct tape, and were not removed from the area by the employer or provided with adequate respiratory protection equipment, such as but not limited to tight fitting respirators with P-100 cartridges.
