29 CFR 1926.102(a)(1): Eye and face protective equipment were not used when machines or operations presented potential eye or face injury from physical, chemical, or radiation agents.  a). At the jobsite - On or about January 7, 2015, an employee was using a Paslode PowerMaster Plus pneumatic nail gun to install roof trusses without the use of eye protection, thus exposing the employee to eye injuries.

29 CFR 1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501(b).  a). At the jobsite - On or about January 7, 2015, employees were working from the second floor top plate installing roof trusses without any fall protection system in place, thus exposing employees to an interior fall hazard of 8.05 feet to wood flooring and a exterior fall hazard of 19.2 feet to earthen ground.

29 CFR 1926.1052(c)(1): Stairways having four or more risers or rising more than 30 inches (76 cm), whichever is less, were not equipped with at least one handrail and one stair rail system along each unprotected side or edge.  a). Inside the Townhome - On or about January 7, 2015, the stairway used to gain access to the second floor was not equipped with a handrail on the closed side and a stair rail system on the unprotected side, thus exposing employees to a trip and a fall hazard of approximately 5 feet to concrete flooring.
