A standard guardrail was not installed around the loading pit, on or about 11/12/14. b) Gwinnett Clean and Beautiful, Duluth, GA - Line #2, employees were exposed to a fall /trip hazard when working around the loading pit area to sort and push recyclable waste. A standard guardrail was not installed around the loading pit, on or about 11/12/14. c) Gwinnett Clean and Beautiful, Duluth, GA - Cardboard baler area, employees were exposed to a fall /trip hazard when working around the loading pit area to sort and push recyclable waste. The standard guardrail installed was damaged on one side and there was no guardrail on the side that employees used to push recyclable waste, on or about 11/12/14. A baseline audiogram had not been conducted, on or about 12/9/14. b) Gwinnett Clean and Beautiful, Duluth, GA - An employee operating the forklift and bobcat was exposed to noise levels at a time weighted average (TWA) of 87.9 dBA, exceeding the action level of 85 dBA, which represented his 8 hour day. A baseline audiogram had not been conducted, on or about 12/9/14.
