29 CFR 1910.23(e)(1): The intermediate rail was not approximately halfway between the top rail and the floor, platform, runway, or ramp:  Employees engaged in material handling activities in the jig storage area, located above the die shop, were exposed to fall hazards of approximately 8 feet from the walking/working surface to the ground below in that the guard railing did not include an intermediate rail approximately half way between the top rail and the floor.

29 CFR 1910.212(a)(1): One or more methods of machine guarding was not provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks:  Employees engaged in jig fabrication activities using the drill press in the wood shop were exposed to struck-by hazards in that machine guarding was not provided.
