29 CFR 1910.133(a)(1): The employer did not ensure that each affected employee used appropriate eye or face protection when exposed to eye or face hazards from flying particles, molten metal, liquid chemicals, acids or caustic liquids, chemical gases or vapors, or potentially injurious light radiation:    On or about 1/12/2015, in the parking lot on the south side of the building, an employee who was trimming tree limbs was not wearing eye protection while exposed to hazards from falling branches and flying particles.

29 CFR 1910.135(a)(1): The employer did not ensure that each affected employee wore a protective helmet when working in areas where there is a potential for injury to the head from falling objects:    On or about January 12, 2015, in the parking lot on the south side of the building, an employee cutting tree limbs overhead without head protection was exposed to head injuries from the falling limbs.
