29 CFR 1910.134(f)(2):  The employer did not ensure that employees using tight fitting face piece respirators pass an appropriate qualitative or quantitative fit test prior to initial use of the respirator, whenever a different respirator facepiece (size, style, model or make) is used, and at least annually thereafter.     (a) On or about 01/13/2015, at facility, for the EMT wearing Kimberly Clark N95 respirator while responding to people with conditions such as, but not limited to, ebola, tuberculosis, and respiratory infections.  The employees were not provided a fit test prior to being required to wear the respirator.
