This violation was observed on or about January 12, 2015, in the fabrication shop near the Femco CNC Lathe where an employee was exposed to struck-by hazards when hoisting material using a 1/2-Ton Coffing overhead hoist without a safety latch on the hoisting hook.   c.  This violation was observed on or about January 12, 2015, in the fabrication shop near the MCV 1250 CNC Lathe where an employee was exposed to struck-by hazards when hoisting material using a 1/4-Ton Coffing overhead hoist without a safety latch on the hoisting hook.   d.  This violation was observed on or about January 12, 2015, in the fabrication shop near the Wintec Horizontal Boring Mill where an employee was exposed to struck-by hazards when hoisting material using a 1-Ton Coffing overhead hoist without a safety latch on the hoisting hook.   e.  This violation was observed on or about January 12, 2015, in the fabrication shop near the Femco WNCL-35 CNC Lathe where an employee was exposed to struck-by hazards when hoisting material using a 1/2-Ton Coffing overhead hoist without a safety latch on the hoisting hook.   f. This violation occurred on or about January 12, 2015, in the front shop area, on the west wall where an employee was exposed to struck-by and caught-by hazards when operating a Moriseiki manual metal lathe machine without a chuck guard.
