29 CFR 1910.303(b)(7)(i): Unused openings in boxes, raceways, auxiliary gutters, cabinets, equipment cases, or housings were not effectively closed to afford protection substantially equivalent to the wall of the equipment.  Employees engaged in production activities in the production shop were exposed to electrical shock and fire hazards in that unused openings in the following locations were not effectively closed:  a)  The junction box on the south side of the four station former included an open knockout on the top and face of the junction box approximately 34 inches above the ground.  b)  An electrical enclosure on the west side of the four station former included an open knockout on the top of the box, approximately 42 inches above the ground.   c)  Two junction boxes in aisle B, above the work desk near the Franco Punch Press, included two open knockouts.    d)  An electrical enclosure housing a dual light switch, on the west wall of the CNC set-up office, included an unused opening where a light switch used to be.
