29 CFR 1910.178(a)(5): The powered industrial truck was equipped with front-end attachments other than factory installed attachments, however the employer did not request that the truck be marked to identify the attachments and show the approximate weight of the truck and attachment combination at maximum elevation with load laterally centered:  On or about January 13, 2015 and times prior to, at the above addressed job site, the employer had a job made fork lift basket and did not ensure the truck had a identification to show the above requirement.
