29 CFR 1926.960(c)(1)(iii): The employer did not ensure that no employee approached or took any conductive object closer to exposed energized parts than the employer's established minimum approach distance:  On or about January 12, 2015, at the sub-station located at 2605 LA Hwy 75, Saint Gabriel, LA, the employer allowed employee(s) testing a voltage meter from a non-insulated aerial lift bucket to approach an energized line of 500KV within the established approach distance of 11ft 3in, exposing the employee(s) to an electrical hazard.
