29 CFR 1926.602(c)(1)(vi): Industrial truck(s) in use did not meet the applicable requirements of design, construction, stability, inspection, testing, maintenance, and operation, as defined in American National Standards Institute B56.1-1969, Safety Standards for Powered Industrial Trucks:  Jobsite located at 4 Pulver Street, Torrington, Connecticut:  On, or about, October 30, 2014, a Clark forktruck, serial number 355-490-4046, tipped over while the operator was attempting to load a 10 foot long can and a three foot diameter drill bit onto a flatbed truck.  The loading operation was conducted on a gravel lot.  A.  The load being extended out beyond the sides of the forks and was not properly secured.  The actual weight of the load was not provided to the operator. The forktruck was provided with hard rubber wheels designed for use on hard surface, such as concrete or pavement.  Ref. ANSI B56.1- 1969, Section 605.A.  B.  The overhead guard provided on the forktruck was bent and damaged.  Ref. ANSI B56.1  1969, Section 702.I.
