29 CFR 1910.217(c)(2)(i)(a): Point of operation guard(s) on mechanical power press(es) did not prevent entry of hands or fingers into the point of operation by reaching through, over, under or around the guard(s): a) Shear Press Area - On or about 1-14-15 - Employee was exposed to amputation hazard from hinged barrier guards that could be opened by the operator while the shears were in operation. There were two Benchmaster Shears in the area with hinged barrier guards on them.

29 CFR 1910.217(c)(2)(i)(d): Point of operation guard(s) on mechanical power press(es) did not utilize fasteners not readily removable by the operator, so as to create minimize the possibility of misuse or removal of essential parts: a) Shear Press Area - On or about 1-14-15 - Employee using a Benchmaster 10 ton shear was exposed to amputation hazards by barrier guards with hinged doors that were not fastened. Hinged doors were not secured with fasteners that were not readily removable by the operator.
