29 CFR 1910.23(c)(1): Open-sided floors and/or platforms four feet or more above adjacent floor or ground level were not guarded with standard railings (or equivalent) and toe boards:    A) Extruder area, on or about January 16, 2015, the top rail for the guardrail system was only 36 inches high for platform on extruder number 2 and there was no chain or a gate across ladder access opening, exposing employees to a fall hazard of approximately 4 feet 4 inches.      B) Extruder area, on or about January 16, 2015, the work platform on extruder number 4, did not have a mid-rail and there was no chain or gate across ladder access opening, exposing employees to a fall hazard of approximately 4 feet 6 inches.      C) Extruder area, on or about January 16, 2015, the work platform on extruder number 5 did not have a mid-rail and there was no chain or gate across ladder access opening, exposing employees to a fall hazard of approximately 4 feet 5 inches.      D) Extruder area, on or about January 16, 2015, the work platform on extruder number 7 did not have a mid-rail and there was no chain or gate across ladder access opening, exposing employees to a fall hazard of over 4 feet.

29 CFR 1910.212(a)(1): One or more methods of machine guarding was not provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks:     A) Maintenance shop, on or about, January 14, 2015, the rotating bit on a Bridgeport milling machine was not guarded, exposing employees to flying materials and possible lacerations from touching the cutting bit.     B) Maintenance shop, on or about, January 14, 2015, the chuck on the Hendey lathe was not guarded exposing employees to the hazard of flying materials and lacerations from the rotating chuck.     C) Maintenance shop, on or about, January 14, 2015, the self-adjusting gravity guard on the Everett Chop saw would not automatically return to and adequately cover the cut off wheel, exposing employee to the hazard of lacerations.       D) Weaving Department, on or about January 14, 2015, weaving machines such as, but not limited to, weaving machine numbers 3, 4, 20, 21, 22, 26, 27, 29, 30, 32, 44, 45, 47, 48, 49, 50, 51, and 52 has a spinning metal bar in the fill carrier that was not guarded, exposing employees to a hazard of lacerations from being struck by the spinning metal bar.      E) Weaving Department, on or about, January 14, 2015, weaving machines such as, but not limited to weaving machine numbers 3, 4, 20, 21, 22, 26, 27, 29, 30, 32, 44, 45, 47, 48, 49, 50, 51, and 52, have rollers that the tape has to run through. These rollers create an in-running nip point that was not guarded, exposing employees to a hazard of fractures.

29 CFR 1910.215(b)(9): The distance between the abrasive wheel periphery(s) and the adjustable tongue or the end of the safety guard peripheral member at the top exceeded one fourth inch:     A) Maintenance shop, on or about January 14, 2015, a Dayton bench grinder did not have a tongue guard for one of the grinding wheels and the tongue guard for the other grinding wheel was approximately 1 inch above the grinding wheel, exposing employees to a hazard of being struck by an exploding grinding wheel.
