29 CFR 1926.451(g)(1): Each employee on a scaffold more than 10 feet (3.1 m) above a lower level were not protected from falling to that lower level. At south side of the structure; an employee worked from a pump jack scaffold that did not have a guardrail system, exposing employees to fall hazards at approximately 22 feet.

29 CFR 1926.452(j)(2): Poles were not secured to the structure by rigid triangular bracing or equivalent at the bottom, top, and other points as necessary. When the pump jack has to pass bracing already installed, an additional brace were not installed approximately 4 feet (1.2 m) above the brace to be passed, and were not left in place until the pump jack has been moved and the original brace reinstalled. At south side of the structure; an employee worked from a pump jack scaffold that was not secured to the wall structure, exposing employees to fall hazards at approximately 22 feet.
