29 CFR 1910.303(b)(2): Listed or labeled electrical equipment was not used or installed in accordance with instructions included in the listed or labeling:    Employee(s) using a vacuum machine that was plugged into a metal 4-plex receptacle box that was attached to a dolly on the east side of bay 3, were exposed to electrical hazards.  The metal receptacle box had a 6 foot power cord , thus violating the UL listing/labeling instructions for a receptacle box designed for fixed installation.
