On or about January 21, 2015 at the above addressed jobsite, two employees were observed installing roofing shingles with a pneumatic nail gun, without wearing protective eyewear, therefore exposing the employees to eye injuries from the nails and pieces of splintered roofing material.

On or about January 16, 2015 at the above addressed jobsite, an employee was using a circular saw to cut roof sheathing (plywood), which had damage to the outer insulation of the electrical power cord, therefore exposing the employee to an electrical hazard.
