29 CFR 1926.1053(b)(1): Portable ladders were used for access to an upper landing surface and the ladder side rails did not extend to at least 3 feet (.9m) above the upper landing surface to which the ladder was used to gain access nor was the ladder secured at it's top to a rigid support and a grasping device, such as a grabrail, provided to assist the employee in mounting and dismounting the ladder:  a) Durham Park Townhomes, Bldgs. 125 through 135 - On or about January 14, 2015 employees used an aluminum extension ladder to gain access to the second story roof of a townhome without the side rails of the ladder being extended at least 3 feet above the upper landing surface nor was the ladder secured at its top to a rigid support, exposing employees to fall hazards of up to approximately 19 feet onto earthen ground.

29 CFR 1926.102(a)(1): Eye and face protective equipment were not used when machines or operations presented potential eye or face injury from physical, chemical, or radiation agents:  a) Durham Park Townhomes, Bldgs. 125 through 135 - On or about January 14, 2015 employees were not wearing eye protection while using pneumatic nail guns to install roof shingles, exposing employees to possible eye injuries from being struck by a nail.  Epifanio Alarcon was previously cited for a violation of this occupational safety and health standard or its equivalent standard [1926.102(a)(1)], which was contained in OSHA inspection number 959649, citation number 1, item number 2 and was affirmed as a final order on June 11, 2014, with respect to a workplace located at 111 Jonesburg Court, Lot 6, Savannah, GA 31405.

29 CFR 1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b):   a) Durham Park Townhomes, Bldgs. 125 through 135 - On or about January 14, 2015, employees installing roof shingles were exposed to fall hazards up to approximately 19 feet onto earthen ground because the workers were not fall protected.  Epifanio Alarcon was previously cited for a violation of this occupational safety and health standard or its equivalent standard [1926.501(b)(13)], which was contained in OSHA inspection number 959649, citation number 1, item number 3 and was affirmed as a final order on June 11, 2014, with respect to a workplace located at 111 Jonesburg Court, Lot 6, Savannah, GA 31405.
