29 CFR 1910.184(c)(6): Slings were not securely attached to their loads:   The employer is failing to protect employees from struck-by hazards in the workplace.  Employees are not ensuring that batteries being lifted by hoist and sling are securely attached prior to lifting.  An employee was injured when the battery being lifted free from the rigging and fell causing the rigging bar to swing up and strike the employee in the face causing the employee to suffer a laceration to the forehead.

29 CFR 1910.305(b)(1)(ii): Unused openings in boxes, cabinets, or fittings were not effectively closed:   The employer is failing to ensure that employees are protected from electrical hazards.  Employees working near or on the electrical junction and switch boxes in the battery charging area are exposed to live parts due missing "knock-out plugs" creating openings in this box.
