29 CFR 1910.178(q)(6): Powered industrial trucks were altered so that the relative positions of the various parts were different from what they were when originally received from the manufacturer, by the addition of extra parts not provided by the manufacturer, or by the elimination of any parts without the manufacturer's approval:  (a) Near Grinding Area - Employees operated a Yale forklift, #9247, where the clamps to secure the propane tank were missing and had been replaced by a bungee cord thereby exposing the employee to a hazard, observed on or about 1-14-15.   Abatement certification required.
