29 CFR 1926.501(b)(10): Each employee engaged in roofing activities on low-slope roofs with unprotected sides and edges 6 feet or more above lower levels, was not protected from falling by guardrail systems, safety net systems, personal fall arrest systems, or a combination of warning line system and guardrail system, warning line system and safety net system, or warning line system and personal fall arrest system, or warning line system and safety monitoring system.  a) For employees working at 4408 Gulf Drive in Holmes Beach, as observed on or about January 6, 2015. Employees were on the roof working without the use of a fall protection system in place. Employees were exposed to a fall hazard of approximately 9 feet 8 inches.

29 CFR 1926.404(b)(1)(ii): On a construction site, where an assured equipment grounding program was not utilized, all 120-volt, single-phase, 15 and 20 ampere receptacle outlets which were not a part of the permanent wiring of the building or structure and which are in use by employees did not have approved ground fault circuit interrupers for personal protection.  a) For employees working at 4408 Gulf Drive in Holmes beach, as observed on or about January 6, 2015. Employees were using power supplied through an extension cord that was not protected by a ground fault circuit interrupter.
