29 CFR 1926.501(b)(13): Each employee engaged in residential construction activities 6 feet or more above lower levels was not protected by guardrail systems, safety net systems, or personal fall arrest systems, nor were employees provided with an alternative fall protection measure under another provision of paragraph 1926.501(b):       (a)WCB Ventures, LLC. at 3100 E. 17th Ave., Denver, CO 80206:  On and before January 16th, 2015, the controlling employer did not ensure that subcontractor employees were protected from fall hazards while constructing three-story multi-family condominiums. Employees were exposed to fall hazards on the first floor included two open landings, one measuring 44 inches wide, and the other measuring 50 inches wide, with a 9.5 foot fall hazard to the basement. The condition exposed subcontractor employees to fall hazards of approximately 9.5 feet.       (b)WCB Ventures, LLC at 3100 E. 17th Ave., Denver, CO 80206:  On and before January 16th, 2015, the controlling employer did not ensure that subcontractor employees were protected from fall hazards while constructing three-story multi-family condominiums. Employees were exposed to fall hazards on the first floor included 5 window openings, with 3 measuring 28 inches wide, and two measuring 32 inches wide, that included the window sill placed at 36 inches high, and did not have an adequate top rail at least 39 inches high. The condition exposed subcontractor employees to fall hazards of approximately 9.5 feet.       (c)WCB Ventures, LLC at 3100 E. 17th Ave., Denver, CO 80206:  On and before January 16th, 2015, the controlling employer did not ensure that subcontractor employees were protected from fall hazards while constructing three-story multi-family condominiums. Employees were exposed to fall hazards on the second floor included an open landing 10 feet long, with a fall hazard of 21 feet to the basement  The condition exposed subcontractor employees to fall hazards of approximately 21 feet.     (d)WCB Ventures, LLC at 3100 E. 17th Ave., Denver, CO 80206:  On and before January 16th, 2015, the controlling employer did not ensure that subcontractor employees were protected from fall hazards while constructing three-story multi-family condominiums. Two employees were exposed to fall hazards while setting trusses on the garage and were walking on the exterior top plates with a fall distance of 9.3 feet.  The condition exposed subcontractor employees to fall hazards of approximately 9.3 feet.

29 CFR 1926.502(b)(13): When guardrail systems were used around holes which were used as access (such as ladderways), they were not provided with a gate, or were not so offset that a person could not walk directly into the hole:    (a)WCB Ventures, LLC at 3100 E. 17th Ave., Denver, CO 80206:  On and before January 16th, 2015, the controlling employer did not ensure that ladderways used as points of access on the first, second, and third floors included guardrails that were offset, or were provided with a gate, so that a person could not walk directly into the holes.  The condition exposed subcontractor employees to fall hazards of up to 21 feet.
