29 CFR 1910.253(b)(4)(iii): Oxygen cylinders in storage were not separated from fuel-gas cylinders or combustible materials (especially oil or grease), a minimum distance of 20 feet (6.1 m) or by a noncombustible barrier at least 5 feet (1.5 m) high having a fire-resistance rating of at least one-half hour:    a) Jobsite, 149 Nichol Avenue, McKees Rocks, PA 15136:  Observed on or about December 22, 2014, compressed gas cylinders containing oxygen were stored directly next to a cylinder of acetylene.  Employees working in the shop area were exposed to a fire hazard due to the improper storage of oxygen and acetylene gas cylinders.

29 CFR 1910.305(g)(1)(iv)(C): Flexible cords and/or cables were run through doorways, windows, or similar openings:      a) Jobsite, 149 Nichol Avenue, McKees Rocks, PA 15136:  Observed on or about December 22, 2014, a flexible cord (110 Volt) used to power a fuel tank on the outside of the building was run through a doorway.  The cord was exposed to wear and tear from vehicles operating in the area.  Employees working in the main shop area were exposed to the hazard of electric shock.

29 CFR 1910.120(m):  While work was in progress, areas accessible to employees were lighted at less than the minimum illumination intensities listed in Table H-120.1:  a) Jobsite, 149 Nichol Avenue, McKees Rocks, PA 15136:  Observed on or about December 22, 2014, employees were not provided adequate lighting while working in the shop area due broken/inoperable overhead lights and broken lights on vehicles operating in the area.  Employees sorting materials in the shop area were exposed to tripping and struck-by hazards due to the lack of adequate lighting.
