29 CFR 1910.178(a)(4): Modifications and additions which affect capacity and safe operation of powered industrial truck were performed by the employer without the manufacturer's prior written approval:  In the welding area: On or about January, 20, 2015 and at times prior thereto, employees were exposed to a struck-by hazard while using Toyota Forklift Model 5FGC15, Serial 74264 with a modified forklift tine (hole in fork) that significantly reduces the lifting capacity of the tine resulting in a possible break of tine and unintentional dropped load.
