29 CFR 1926.451(b)(2): Scaffold platforms and walkways were not at least 18 inches wide:  a) At the job site, bldg. 3500, on or about January 21, 2015, two employees were exposed to fall hazard while using a 2 X 10 and 2 X 6 wooden platforms as a walkway, exposing employees up to a 16-foot fall hazard.

29 CFR 1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b):  29 CFR 1926.501(b)(1): Each employee on a walking/working surface with an unprotected side or edge which was 6 feet (1.8 m) or more above a lower level was not protected from falling by the use of guardrail systems, safety net systems, or personal fall arrest systems:  a) At the job site, bldg. 3500, on or about January 21, 2015, two employees were observed working from an unguarded 2nd floor balcony, exposing employees to a 10 foot hazard.

29 CFR 1926.1053(b)(1): Where (a) portable ladder(s) was/were used for access to an upper level landing surface and the ladder's length did not allow the ladder side rails to extend at least 3 feet (0.9 m) above the upper landing surface being accessed, the ladder was not secured at its top to a rigid support that will not deflect, and/or a grasping device was not provided to assist employees in mounting and dismounting the ladder:     a) At the job site, bldg. 3500, on or about January 21, 2015, a wooden job made ladder did not extend 3-feet beyond the balcony deck, exposing employees to a 10-feet 8-inch fall hazard.
