29 CFR 1910.147(c)(1):  The employer did not establish a program consisting of an energy control procedures, employee training and periodic inspections to ensure that before any employee performed any servicing or maintenance on a machine or equipment where the unexpected energizing, start up or release of stored energy could occur and cause injury, the machine or equipment would be isolated, and rendered inoperative in accordance with 29 CFR 1910.147(c)(4):     (a)  Byers Industries, Inc. at 810 S. Sherman St., Longmont, CO 80501:  On and before January 14, 2015, the employer did not establish a program consisting of an energy control procedure and employee training to ensure that before any employee performed any servicing or maintenance on a machine or equipment where the unexpected energizing, start up or release of stored energy could occur and cause injury, the machine or equipment would be isolated and rendered inoperative, including but not limited to, the fifteen plastic injection mold machines in the production area.

Sherman St., Longmont, CO 80501:  On and before January 14, 2015, the employer failed to provide a workplace free from recognized electrical hazards in that electrical cords had exposed inner insulation. This condition exposed the employees to the hazard of electrical shock for the following equipment:     1)     220 Volt power cord for 2-Ton crane at the plug end.  2)     220 Volt power cord for 2-Ton crane at the motor end.  3)     220 Volt power cords to various dryers.  4)     Various 120 Volt drop power cords in production area.

Sherman St., Longmont, CO 80501:  On and before January 14, 2015, the employer failed to maintain adequate strain relief on the multiple conductor cable that controls the 2-Ton crane in the production area. The electrical cord outer sheath had pulled away from the pendant and had exposed inner insulation. This condition exposed the employees to the hazard of electrical shock.
