29 CFR 1926.403(b)(1)(iii): Safety of equipment was not determined on the basis of electrical insulation. a- Building A - Modular Home Section 27501-A, 301 Riverside Drive, Millville NJ: On or about 01/21/15 a company employee performing sheetrock installation and other related activities, while using electric powered tools, was exposed to electrocution hazards when utilizing two separate extension cords whose outer covering were visibly damaged. NOTE: IN ADDITION TO ABATEMENT CERTIFICATION, THE EMPLOYER IS REQUIRED TO SUBMIT ABATEMENT DOCUMENTATION FOR THIS ITEM, FAILURE TO COMPLY WILL RESULT IN AN ADDITIONAL PENALTY OF $1000.00 IN ACCORDANCE WITH 29 CFR 1903.19.
