29 CFR 1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b):   (a)  At the site - employee was not provided fall protection while walking the concrete block wall, on or about 01/22/2015.  (b)  At the site - employee was not provided fall protection while performing concrete work at the wall opening, on or about 01/22/2015.

29 CFR 1926.701(b): All protruding reinforcing steel, onto and/or into which employees could fall or come against, was not guarded to eliminate the hazard of impalement:  (a)  At the site - rebar protruding from the concrete wall that the employee was walking was not protected exposing employee to an impalement hazard, on or about 01/22/2015.
