29 CFR 1926.453(b)(2)(iv): Employee(s) working in an aerial lift were not standing firmly on the floor of the basket:    On the jobsite located at the Bosselman Petroleum Equipment 4705 Jurgens Road, Grand Island, NE 68803: The employer failed to ensure that employees operating a Genie GS-26668 RT (serial Unknown) stood firmly on the floor of the basket. On or about January 22, 2015 an employee of Wolf's Drywall, Inc. was observed climbing on the guardrails of a scissor lift to access a roof where work was being performed.

29 CFR 1926.760(a)(1): Each employee engaged in a steel erection activity who is on a walking/working surface with an unprotected side or edge more than 15 feet (4.6 m) above a lower level was not protected from fall hazards by guardrail systems, safety net systems, personal fall arrest systems, positioning device systems, or fall restraint systems.    On the jobsite located at 4507 Jurgens Road, Grand Island, NE 68803: The employer failed to ensure that employees engaged in steel erection activities were protected from fall hazards while working near an unprotected edge greater than 15 feet above a lower level. On or about January 22, 2015 an employee of Wolf's Drywall, Inc was observed installing a steel framed parapet wall on the roof of the Bosselman Petroleum Equipment. The employer did ensure that the employee was protected from the 23 foot fall hazard by guardrails, safety nets, personal fall arrest system or any other means.

29 CFR 1926.761(b): The employer did not train each employee exposed to a fall hazard in accordance with the requirements of 29 CFR 1926.761:    At the jobsite located at 4507 Jurgens Road, Grand Island, NE 68803:  The employer is failing to provide a training program to ensure that employees are able to recognize and minimize hazards when exposed to falls greater than 15 feet. On or about January 22, 2015 an employee was performing steel erection/construction activities while exposed to fall heights of approximately 23 feet without protective measures such as but not limited to guardrails, safety nets and or personal fall protection systems.
