29 CFR 1926.451(g)(1): Each employee on a scaffold more than 10 feet above a lower level were not protected from falling to that lower level.    On or about January 22, 2015, employees working from an elevated forklift basket at a height greater than ten feet were climbing on the guardrail system of the basket thus exposing them to fall hazards.

On or about January 22, 2015, employees installing trusses from the exterior wall top plates and truss webbing at a height greater than six feet were doing so without fall protection.
