29 CFR 1926.501(b)(1): Each employee on a walking/working surface with an unprotected side or edge which was 6 feet (1.8 m) or more above a lower level was not protected from falling by the use of guardrail systems, safety net systems, or personal fall arrest systems: The employer does not ensure that each employee on a walking/working surface with an unprotected side or edge which is 6 feet or more above a lower level is protected from falling by the use of guardrail systems, safety net systems, or personal fall arrest systems. This violation was observed on or about January 21, 2015, at the job site where an employee was exposed to a 14 foot, 7 inch fall when installing the facade without being protected from falling to the lower level by the use of a fall protection system. Pursuant to 29 C.F.R. 1903.19, within ten (10) calendar days of the abatement date, the employer must submit documentation showing that it is in compliance with the standard, including describing the steps that it is taking to ensure that employees are protected from falling by the use of a fall protection system.

29 CFR 1926.1053(b)(1): Where portable ladders were used for access to an upper landing surface and the ladder's length allows, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface being accessed: The employer does not ensure that the ladder side rails extend at least 3 feet above the upper landing surface to which the ladder is used to gain access. This violation was observed on or about January 21, 2015, at the job site where an employee was using a portable ladder to gain access to the working surface of the awning without the ladder side rails extending at least 3 feet past the upper landing surface. Pursuant to 29 C.F.R. 1903.19, within ten (10) calendar days of the abatement date, the employer must submit documentation showing that it is in compliance with the standard, including describing the steps that it is taking to ensure that the ladder side rails extend at least 3 feet above the upper landing surface to which the ladder is used to gain access.
