29 CFR 1926.405(b)(1): Unused openings in cabinets, boxes, and fittings were not effectively closed:   a. The employer did not ensure that unused openings in a panel board was effectively closed.  This violation occurred on or about January 23, 2015 when the employer allowed live electrical parts to be exposed due to a missing circuit breaker at a temporary panel on Level 1 on the right side of the stage.   b. The employer did not ensure that unused openings in a panel board was effectively closed.  This violation occurred on or about January 23, 2015 when the employer allowed live electrical parts to be exposed due to a missing circuit breaker at a temporary panel on Level 1 at the front of the stage.

This violation occurred on or about January 23, 2015 when a hole in the floor to be used for HVAC and electrical systems on Level 1 was not protected by covers or guardrails.  b. The employer did not ensure that each employee on walking/working surfaces was protected from falling through holes more than six (6) feet above lower levels.  This violation occurred on or about January 23, 2015 when a hole for the elevator shaft on Level 3 was not protected by covers or guardrails.
