29 CFR 1926.453(b)(2)(iv): Employees were not standing firmly on the floor of the basket, and shall not sit or climb on the edge of the basket or use planks, ladders, or other devices for a work position:  a) Genie S-40 basket: On or about January 21, 2015, the employer did not ensure an employee using and working from the basket did not stand on the mid-rail of the guardrail system which exposed the employee to a fall of approximately 13-feet to the ground below.

29 CFR 1926.453(b)(2)(v): A body belt was not worn and/or a lanyard was not attached to the boom or basket when working from an aerial lift:   a) Genie S-40 basket: On or about January 22, 2015, the employer did not ensure an employee using and working from the basket was protected by a personal fall arrest system which exposed the employee to falls up to 14-feet to the ground below.
