29 CFR 1904.39(b)(2): The employer did not report the in-patient hospitalization, amputation, or loss of an eye to OSHA within twenty-four (24) hours after a work-related incident resulting in the in-patient hospitalization:   1. Yard - The employer did not report a work-related, in-patient hospitalization to OSHA within 24 hours of the event.  On January 17, 2015, an employee reversed a forklift truck onto a scrap radiator and accidently pushed it into the leg of another employee.  This employee was hospitalized from January 17, 2015 to January 22, 2015 at Aria Health Frankford Torresdale Hospital.  The employer called the OSHA hotline on January 19, 2015.  Observed On or About January 23, 2015
