Fabricated frame scaffolding at east and northeast sides of the building.  Employees were not tied off while dismantling fabricated frame scaffolding and were exposed to a fall hazard of approximately 30 feet.

29 CFR 1926.454(b): The employer did not have each employee involved in erecting, disassembling, moving, operating, repairing, maintaining, or inspecting a scaffold trained by a competent person to recognize any hazards associated with the work in question.  Jobsite:  Scaffolding at east and northeast sides of Building B  Scaffold erectors /  dismantlers were not adequately trained in fall hazards and fall protection.
