29 CFR 1910.134(f)(2): Employee(s) using a tight-fitting facepiece respirator were not annually fit tested:    a) Wire Weld, Inc. dba Cost Effective Coatings, Inc. / 12069 East Main Road, North East, PA: On or about January 23, 2015 and times prior thereto, employees utilized tight-fitting 3M brand half mask respirators with P95 filters/cartridges during clean up operations.  The employer did not conduct annual fit testing with employees required to use the respirators.
