29 CFR 1910.178(q)(1): Any power-operated industrial truck not in safe operating condition was not taken out of service, and/or repairs were not made by authorized personnel:  a. On or about January 15, 2015, at the concrete plant, the John Deer forklift, model# 444H, did not have a functional back-up alarm, exposing employees to a crushing hazard.  b. On or about January 15, 2015, at the concrete plant, the Caterpillar forklift, serial # did not have a functional horn, exposing employees to a crushing hazard.
