OSH ACT of 1970 Section (5)(a)(1):   The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm including severe burns and explosions from accumulated saw dust in and around ignition sources without the dust collection system fan or blower on the clean air side of the dust collector and no spark detection with an extinguishing system, or spark detection with an abort gate for duct work or a risk analysis for the dust collection system.  a) Moulder shop:  Employees were exposed to fire and explosion hazards due to accumulated wood dust over 1/32 inches deep, with a KsT value of 23.97 bar meters per second on top of electrical boxes of the chain for the moulder saw that had opening in the box.

Plastic pipe was used in above ground locations throughout the facility to transport compressed air from compressor through one half inch PVC lines at 110 psi.

29 CFR 1910.95(c)(1): The employer shall administer a continuing, effective hearing conservation program, as described in paragraphs (c) through (o) of this section, whenever employee noise exposures equal or exceed an 8-hour time-weighted average sound level (TWA) of 85 decibels measured on the A scale (slow response) or, equivalently, a dose of fifty percent. For purposes of the hearing conservation program, employee noise exposures shall be computed in accordance with appendix A and Table G-16a, and without regard to any attenuation provided by the use of personal protective equipment.  a)  Molding/Milling Machine Operator:   On or about February 3rd, 2015 an employee was exposed to noise levels at the hearing conservation level of 117.2 % (equivalent to an 8 hour time weighted average TWA of 91 dBA) which is 2.34 times the action level of 50% (equivalent to an 8 hour TWA of 85 dBA) while operating the molder/milling machine. The sampling period was for 453 minutes with the remainder of the 8 hour shift given a 0 db for the 8 hour Time Weighted Average calculation.  b)  Molding/Milling Machine Conveyor Operator:   On or about February 3rd, 2015 an employee was exposed to noise levels at the hearing conservation level of 116.5 % (equivalent to an 8 hour time weighted average TWA of 91 dBA) which is 2.33 times the action level of 50% (equivalent to an 8 hour TWA of 85 dBA) while unloading finished wooden boards off of the molder/milling machine conveyor. The sampling period was for 457 minutes with the remainder of the 8 hour shift given a 0 db for the 8 hour Time Weighted Average calculation.
