29 CFR 1910.334(a)(2)(i): Portable cord and plug connected equipment and flexible cord sets (extension cords) were not visually inspected before use on any shift for external defects (such as loose parts, deformed and missing pins, or damage to outer jacket or insulation) and for evidence of possible internal damage (such as pinched or crushed outer jacket):    a) In the Shipyard area, on Barge #82, a fuel barge, on the 110 volt power cord for the sump pump, the ground plug was missing.        b) Shop Area, a yellow flexible 110 power cord outer insulation was pulled out of the plug, allowing strain to be placed on its inner electrical connections.

29 CFR 1915.73(d): When employees were exposed to unguarded edges of decks, platforms, flats, or similar flat surfaces, more than 5 feet above a solid surface, the edges were not guarded by adequate guardrails meeting the requirements of 29 CFR 1915.71(j)(1) and (2):               a)    In the Shipyard Area, on the south barge surface, 15 feet above the ground, employees were not protected from fall hazards by use of guard rails.
