29 CFR 1910.151(c): Where employees were exposed to injurious corrosive materials, suitable facilities for quick drenching or flushing of the eyes and body were not provided within the work area for immediate emergency use: On or about January 23, 2015, in the chemical cage, where highly corrosive chemicals including, but not limited to, Enviro Klor HD (pH 14.0) were dispensed from drums and totes into open pails by contract employee(s), facilities for quick drenching or flushing of the eyes and body were not accessible for immediate emergency use within the work area. Travel around spill containment pallets for drums/totes and the long gate was required to reach the eye wash outside the chemical cage, and the nearest emergency shower/eyewash was located more than 50 feet away in the battery charging area, where employee(s) would have to travel around forklifts and pallet jacks, exposing employees to the hazards of corrosive chemical splashes.

29 CFR 1910.157(c)(1): Portable fire extinguishers were not mounted, located and identified so that they were readily accessible without subjecting the employees to injuries: On or about January 23, 2015, in the raw cooler hallway, where employees were charging batteries on forklifts and pallet jacks, and flammable liquid sanitizer was handled by contract employees, Amerex #11 portable fire extinguisher was blocked by stored equipment including a broken pallet jack and portable hoist, exposing employees to the hazards of fire.
