29 CFR 1910.151(c): Where employees were exposed to injurious corrosive materials, suitable facilities for quick drenching or flushing of the eyes and body were not provided within the work area for immediate emergency use:    On or about January 23, 2015, in the chemical cage, where highly corrosive chemicals including, but not limited to, Enviro Klor HD (pH 14.0) were dispensed from drums and totes into open pails by contract employee(s), facilities for quick drenching or flushing of the eyes and body were not accessible for immediate emergency use within the work area. Travel around spill containment pallets for drums/totes and the long gate was required to reach the eye wash outside the chemical cage, and the nearest emergency shower/eyewash was located more than 50 feet away in the battery charging area, where employee(s) would have to travel around forklifts and pallet jacks, exposing employees to the hazards of corrosive chemical splashes.

29 CFR 1910.157(c)(1): Portable fire extinguishers were not mounted, located and identified so that they were readily accessible without subjecting the employees to injuries:    On or about January 23, 2015, in the raw cooler hallway, where employees were charging batteries on forklifts and pallet jacks, and flammable liquid sanitizer was handled by contract employees, Amerex #11 portable fire extinguisher was blocked by stored equipment including a broken pallet jack and portable hoist, exposing employees to the hazards of fire.

29 CFR 1910.303(g)(1)(i)(A): For electric equipment operating at 600 volts, nominal, or less to ground, and likely to require examination, adjustment, servicing, or maintenance while energized, the depth of the working space in the direction of access to live parts (as measured from the live parts where they were exposed or from the enclosure front or opening where they were enclosed) was less than indicated in Table S-1 of Subpart S of Part 1910:    On or about January 23, 2015, raw cooler area, Square D Power Panel DP 480-Volt box for the freezer control panel, lighting, and battery chargers and Lighting Panel DL 240/120 V box controlling lighting were blocked by stored equipment including a broken pallet jack and portable hoist.
