29 CFR 1910.305(b)(1)(ii): Unused openings in boxes, cabinets, or fittings were not effectively closed.  On or about January 29, 2015, in the mechanic room, the employer allowed employees who were using a breaker in a 480 volt panel board to turn on/off the shop lights to be exposed to a missing breaker in the panel board leading to an unused opening that was not covered.
