29 CFR 1910.305(b)(2)(i): Each outlet box in completed installations did not have a cover, faceplate, or fixture canopy:  In the following instances outlet boxes were not equipped with faceplates:  a) Steel Room - Broken cover above table magnet.  b) Steel Room - Missing cover on corner outlet.  c) Packaging Area - Two outlets were missing covers.  d) Steel & Fabrication Building - Broken cover on heating unit outlet.  DeWitt Machine & Fabrication was previously cited for a violation of this occupational safety and health standard, which was contained in OSHA inspection number 314960485, Citation 1, Item 2 and was affirmed as a final order on 12/1/2010.
