29 CFR 1926.102(a)(1): Employees shall be provided with eye and face protection equipment when machines or operations present potential eye or face injury from physical, chemical, or radiation agents:   (a)  On or about January 26, 2015, employees were exposed to eye injuries resulting from utilizing pneumatic nailers while installing shingles without the benefit of eye protection.   Note:  Carmona Roofing, LLC was previously cited for a violation of this Occupational Safety and Health Standard 29 CFR 1926.102(a)(1) or its equivalent which was contained in OSHA inspection 585840, Citation 1, Item 1, issued September 6, 2012, with respect to a workplace located at 2023 Tesla Drive, Colorado Springs, CO 80909.  This citation became final on September 28, 2012.

29 CFR 1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b):   (a)  On or about January 26, 2015, employees were exposed to injuries from falls, while conducting reroofing activities, without the benefit of fall protection systems being in place, on a roof in excess of six feet above the lower surface.  Note:  Carmona Roofing, LLC was previously cited for a violation of this Occupational Safety and Health Standard 29 CFR 1926.501(b)(13) or its equivalent which was contained in OSHA inspection 585840, Citation 1, Item 2, issued September 6, 2012, with respect to a workplace located at 2023 Tesla Drive, Colorado Springs, CO 80909. This citation became final on September 28, 2012.
