29 CFR 1926.50(g): Where employees were exposed to injurious corrosive materials, suitable facilities for quick drenching or flushing of the eyes and body were not provided within the work area for immediate emergency use: (a) Worksite - Lead abatement workers were exposed to eye injury hazards in not having an emergency eyewash available while applying and removing the corrosive chemical product Peel Away 1, on February 4, 2015.

29 CFR 1910.134(c)(2)(i): Respirator users were not provided with the information contained in Appendix D to 29 CFR 1910.134 when the employer determined that any voluntary respirator use was permissible: (Construction Reference 1926.103) (a) Worksite - Lead abatement workers permitted to wear 3M Model 7700 half-face respirators while performing lead abatement operations were not provided with Appendix D information regarding their use, on or about January 29, 2015. (b) Worksite - Demolition workers permitted to wear N95 Model filtering face piece respirators while performing demolition operations were not provided with Appendix D information regarding their use, on or about January 29, 2015. Abatement certification required within 10 days after abatement date. The certification shall include a statement that abatement is complete, the date and method of abatement, and state that employees and their representatives were informed of this abatement.
