B) Back right corner of shop, on or about, January 28, 2015, the cutting blade on a shearing machine was not guarded, exposing employees to a hazard of amputations to fingers or hands. C) Shop, on or about, January 28, 2015, an Accurpress press brake and a Brakemaster press brake were not guard or other means taken to prevent employees from having their fingers or hands in the area between the die and the punch during operation. D) Front left of Shop, on or about, January 28, 2015, the foot pedal that activates a Brakemaster press brake was not guarded, exposing employees to an amputation hazard. E) Back middles section of shop area, on or about January 28, 2015, the shearing blade on a Scotchman Iron worker metal cutting machine was not properly guarded, exposing employees to a hazard of amputations to fingers.
