29 CFR 1926.403(b)(2): 29 CFR 1926.403(b)(2): Listed, labeled, or certified equipment was not installed and used in accordance with instructions included in the listing, labeling, or certification:  a. 175 Quaker Church Rd, Randolph, NJ  Employees engaged in commercial construction activities used a relocatable power tap as a source of power for multiple cords and in a manner that was not recommended by the manufacturer. 29 CFR 1926.405(g)(2)(iv): Flexible cords were not connected to devices and fittings so that strain relief is provided to prevent pull from being directly transmitted to joints or terminal screws:  a. 175 Quaker Church Rd, Randolph, NJ  Strain relief was not provided on flexible cord to prevent pull from being transmitted directly to the cord joints. Employees were observed using flexible cords without strain relief to power portable hand tools.    Violation observed on or about January 29, 2015
