29 CFR 1926.454(a): The employer did not have each employee who performed work while on a scaffold trained by a person qualified in the subject matter to recognize the hazards associated with the type of scaffold being used and to understand the procedures to control or minimize those hazards.  Each bricklayer installing brick masonry units from a supported scaffold at an elevation of 13 feet above ground level were not trained to recognize trip and fall from height hazards.

29 CFR 1926.451(b)(1):     Each platform on all working levels of scaffolds was not fully planked or decked between the front uprights and the guardrail supports as specified in paragraphs 1926.451(b)(1)(i)-(ii):    The Robinson Masonry LLC was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1926.451(b)(1), which was contained in OSHA inspection number 874523, citation number 1, item number 3-a and was affirmed as a final order on 04/24/2013, with respect to a workplace located at 86 Tournay Circle, Little Rock, Arkansas 72212.    Employees worked from a supported scaffold to perform masonry activities which was not fully planked.

29 CFR 1926.451(g)(4)(i): Guardrail systems were not installed along all open sides and ends of platforms before being released for use by employees other than erection/dismantling crews:   Employees were working from a supported scaffold which did not have a guardrail system installed along all open sides and ends of the scaffold and Personal Fall Arrest Systems (PFAS) were not utilized as an additional means of protection.
