29 CFR 1926.452(b)(2): Transverse bracing forming an "X" across the width of the scaffold was not installed at the scaffold ends and at least at every third set of posts horizontally (measured from only one end) and every fourth runner vertically. Bracing shall extend diagonally from the inner or outer posts or runners upward to the next outer or inner posts or runners. Building ties shall be installed at the bearer levels between the transverse bracing and shall conform to the requirements of 1926.451(c)(1).  (a).  Pooler construction site, on or about and at times preceding to January 30, 2015, masonry scaffolding did not include proper cross bracing to ensure stability of scaffold, exposing employees to a 10 foot fall hazard.

29 CFR 1926.451(e)(1): When scaffold platforms were more than 2 feet (0.6 m) above or below a point of access, portable ladders, hook-on ladders, attachable ladders, stair towers (scaffold stairways/towers), stairway-type ladders (such as ladder stands), ramps, walkways, integral prefabricated scaffold access, or direct access from other scaffold, sturcure, personnel hoist, or similar surface was not used.  Crossbraces were used as a means of access.  (a). Pooler construction site, on or about and at times preceding to, January 30, 2015, tubular welded masonry scaffold was not equipped with an access ladder for employees laying brick in the door archway.
