29 CFR 1926.102(a)(1): Eye and face protective equipment were not used when machines or operations presented potential eye or face injury from physical, chemical, or radiation agents.  On or about January 19, 2015 and at times prior thereto,  employees performed roofing construction with pneumatic nail guns on the second story roof of a single family residential construction without using eye protection.     Abatement certification and documentation required within 10 days after abatement date. The certification shall include a statement that abatement is complete, the date and method of abatement, and state that employees and their representatives were informed of this abatement. Abatement documentation shall include documents demonstrating that abatement is complete, such as evidence of the purchase or repair of equipment, photographic or video evidence of abatement or other written records.

29 CFR 1926.404(b)(1)(ii): On a construction site, where an assured equipment grounding program was not utilized, all 120-volt, single-phase, 15 and 20 ampere receptacle outlets which were not a part of the permanent wiring of the building or structure and which are in use by employees did not have approved ground fault circuit interrupers for personal protection.  On or about January 29, 2015 and at times prior thereto, employees were performing  home renovation work using electrical extension cords and circular saws without ground fault circuit interrupters.

29 CFR 1926.1051(a): Stairways or ladders were not provided at all personnel points of access where there was a break in elevation of 19 inches (48 cm) or more, and no ramp, runway, sloped embankment, or personnel hoist was provided.  a) On or about January 29, 2015 and at times prior thereto, the patio that led to the entrance of the construction site used by employees was greater than 27 inches above the ground.      b) On or about January 29, 2015 and at times prior thereto, employees used an unopened ladder on the second floor,  to climb halfway up the wall, at which point the employees would climb the framing of the house until they reached an opening in the ceiling that led to the roof.  Abatement certification required within 10 days after abatement date.  The certification shall include a statement that abatement is complete, date completed, method of abatement, and states employees and their representatives were informed of this abatement.
