29 CFR 1910.132(a): Protective equipment , including personal protective equipment for eyes, face, head, and extremities, protective clothing, respiratory devices, and protective shields and barriers, was not used wherever it is necessary by reason of hazards of processes or environment: On October 7, 2014, in the Chlor-Alkali Unit at this facility, an employee was exposed to chemical burns when de-inventorying hazardous chemicals from a column and the employee did not utilize personal protective equipment, such as, but not limited to, a chemical acid suit.

29 CFR 1910.151(c): Where employees were exposed to injurious corrosive materials, suitable facilities for quick drenching or flushing of the eyes and body were not provided within the work area for immediate emergency use: At this facility, Chlor-Alkali Unit, on October 7, 2014, employees unloaded sulfuric acid from columns into vacuum trucks and from vacuum trucks to poly storage tanks. Two employees were splashed with sulfuric acid over their bodies. The employer did not ensure an eyewash station was kept in a suitable operating condition for emergency use.
