29 CFR 1926.102(a)(1): Eye and face protective equipment were not used when machines or operations presented potential eye or face injury from physical, chemical, or radiation agents:  (a) Roof, new residential construction located at 5235 Brookhollow Drive, Douglasville, GA 30135: Employees were exposed to eye injuries while operating Hitachi (Model NR83AS1) 3-1/4-inch Framing Nail Guns at 120 PSI with no protective eyewear.

29 CFR 1926.501(b)(13): Each employee engaged in residential construction activities 6 feet (1.8 m)  or more above lower levels was not protected by guardrail systems, safety net systems, personal fall arrest systems, or an alternative fall protection measure under another provision of paragraph 1926.501 (b), nor did the employer demonstrate that it is infeasible or creates a greater hazard to use these systems and develop and implement a fall protection plan which meets the requirements of paragraph (k) of section 1926.502:   (a) Roof, new residential construction located at 5235 Brookhollow Drive, Douglasville, GA 30135: Employee was exposed to falls while installing 1/2-inch plywood sheathing on the rear 10/12 pitched roof at eave heights of approximately 9-feet, 9-inches with no fall protection system in place.  (b) Roof, new residential construction located at 5235 Brookhollow Drive, Douglasville, GA 30135: Two employees were exposed to falls while engaged in residential framing activities on the 4/12 pitched front lower roof at heights of approximately 10-feet with no fall protection system in place.
